Registration is an important step to becoming a Burgess patient. It helps correctly identify you and your medical information. Registration includes collecting basic information from you and creating your medical record.
Please bring these items to every appointment:
- All active insurance cards, including Medicare and/or Medicaid cards

Picture ID (such as your driver’s license)
- Social Security number
- Any paperwork given to you by your physician
- Current medication list (including herbal and over-the-counter drugs) and the name and dosage of each medication, or bring the actual medication bottles with you

A list of your current medical conditions

A list of your last immunizations such as flu, pneumonia, and tetanus

A list of your known allergies, including those to medications and anesthesia

Important phone numbers
- Living Will, if applicable
- Advance Directives, if applicable
- Powers of Attorney, if applicable
